# Why is my policy cancelled?

## A policy can be cancelled for several reasons. Please see below for more information.

**Nonpayment:** The required payment was not received.

**Fraud or misrepresentation:** An issue was identified with the accuracy of the information provided for the policy.

**Safety compliance:** Required safety or inspection requirements were not met.

**Flat nonrenewal:** The required deposit was not received before the renewal period.

**Flat cancellation:** The policy is voided back to its effective date, as though it never took effect. This is typically requested by the agent or policyholder when coverage was not needed, duplicate coverage exists, or the policy was issued in error.

**Voluntary cancellation / LPR:** You or your agent requested cancellation. Unlike a flat cancellation, which voids the policy back to its effective date, this type of cancellation requires a signed LPR form from the insured.
