If you believe there is an error in your premium audit results, you may submit a formal dispute for review by the Premium Audit Department. Beacon Mutual will review disputes that are submitted in writing and include supporting documentation. Before You Submit a Dispute To identify the auditor who completed your audit: 1. Enter your policy number, policy name, or Beacomp account number in the **Find Policy** field. 2. Select the **Audit Detail** tab. 3. Open the most recent audit assignment listed for the policy. 4. Review the assigned auditor information. How to Submit a Dispute Your dispute must be submitted as a written letter or email and must be: - Signed by a principal of the company - Addressed to the Premium Audit Department - Directed to the auditor who completed the audit What Should Be Included? To help ensure a timely review, include: - Policy number - Business name - Detailed explanation of the concern - Supporting records or documentation - Contact information for follow-up questions **Can I dispute an audit over the phone?** No. Audit disputes must be submitted in writing and signed by a principal of the company. **What documentation should I include?** Include any payroll records, tax filings, subcontractor documentation, corporate officer information, or other records that support your dispute. The Premium Audit Department will review the information provided. **How do I know who completed my audit?** Your assigned auditor is listed in the Audit Detail section of your policy record.
