What Documents Are Required for a Premium Audit?

During a premium audit, you may be asked to provide payroll and tax records that support the wages reported during the policy period.

Commonly Requested Documents:

  • Gross Payroll Report covering the full policy period
  • Rhode Island TX-17 filings for the four quarters that correspond with the policy period
  • Federal Form 941 filings for the four quarters that correspond with the policy period

Why Are These Documents Needed? The audit process helps verify payroll and ensure wages are assigned to the correct workers' compensation classifications.

Contact your assigned auditor as soon as possible. The auditor will review your situation and determine whether an extension may be appropriate.

I am not sure what records are being requested.
