How Do I Dispute My Premium Audit Results?

If you disagree with your audit results:

• Submit your dispute within 30 days of receiving the audit.
• Include details about what you are disputing (e.g., employee classifications, codes, subcontractors).
• For onine audit, send disputes to beaconmutualauditdisputes@zoomaudits.com
• For all other audit types mail/fax a signed letter to Beacon Mutual’s Premium Audit Department at
