How to Report an Injury

By submitting a claim online or by calling us, Beacon will submit the First Report of Injury on your behalf.

By law, all workplace injuries must be reported — even if you dispute the legitimacy of the claim. The sooner an injury is reported, the quicker Beacon can help and begin a thorough investigation, which may include speaking with you and other involved parties.

When should an injury be reported?

  • Immediately after you become aware of the injury, even if no medical treatment is needed at the time.
  • All alleged work-related injuries must be reported, including incidents you believe may not be compensable. Reporting creates a record and protects all parties if treatment is later required.

 Submit Your First Report of Injury by Policy

Report your claims 24/7, when it is convenient for you. By reporting online, you will receive an immediate confirmation with the claim number and the claim representative’s name, phone number and email address.


Beacon Mutual Policies

To report a claim online, you need an Online Portal account. If you do not have an account or see the First Report of Injury option, your organization’s key contact can create your account and assign the role needed to report workplace injuries.

Argo Group Policies

Employers with an Argo Group policy can report workplace injuries directly to Argo using either their online reporting tool or dedicated claims phone line.

    What information do I need to report an injury?

    You should be prepared to provide the following information when reporting a claim:
     
    Employer Information
    • Company name
    • FEIN
    • Beacon account number
    • Policy number
    Injured Worker Information
    • Full name
    • Social Security number
    • Address
    • Date of birth
    • Telephone number
    Injury Details
    • Date, time, and location of the injury
    • How the injury occurred
    • Type of injury (burn, cut, strain, etc.)
    • Body part affected
    Medical & Witness Information
    • Name and address of the doctor or medical facility
    • Witness names and contact information (if applicable)

    What happens after I report an injury?

    • Beacon creates an official claim record and assigns a claim number.
    • For lost‑time claims, a Beacon claim representative will contact you, typically within 24 hours after the claim is reported.
    • Medical‑only claims may not require follow‑up unless additional information is needed.
